Completion of Remittance Forms

As Of May 1, 2018 all remittance forms MUST be completed with all required information. The following information must be included for all remittances.

Failure to do so will result in an administrative fee of $400.00 being charged per report.

Reporting Employee Wages

All hours are required to be reported by the 30th day of the month following the month in which the hours were worked. (Example : Hours worked in March are to be reported and paid by April 30th)
